RI 10

Just a quick update before the Christmas holidays - and a chance to wish you all a very Merry Christmas and a Happy New Year!

Cécile had her tenth re-induction yesterday. Only two more to go now! She's back on corticosteroids for the week which isn't much fun but at least she'll have finished before Christmas Day so no holding back on turkey and stuffing!

She has her ups and downs but apart from getting tired quickly she's living a pretty normal life. School is fine - she's had an absolutely glowing report with very positive comments from her teachers. She's looking forward to the two-week break and seeing her brothers at Christmas.

The next appointment is her quarterly check-up with her haematologist early January. Until then she'll be making the most of her holidays. Happy Christmas everyone!

Christmas Cake!

Une dernière mise à jour avant les vacances - et l'occasion pour nous de vous souhaiter à tous un Joyeux Noël et une très bonne année 2016!

Cécile était à l'hôpital hier pour sa dixième ré-induction. Il n'en reste plus que deux! Elle commence sa semaine de corticoïdes, ce qui veut dire les contraintes alimentaires et les douleurs articulaires ... mais elle aura fini avant Noël et ne sera pas privée de tous les repas de fêtes!

Globalement tout se passe bien et à part la fatigue chronique elle mène une vie plutôt 'normale'. Elle réussit bien sa seconde - son bulletin était excellent avec des commentaires élogieux de ses enseignants. Elle attend les vacances avec impatience et surtout les retrouvailles avec ses frères.

Son prochain rendez-vous aura lieu début janvier avec son médecin référent à Debré. Mais pour l'instant elle va profiter de ses vacances. Joyeux Noël à tous!

RI 9

Today is Saint Cecile's Day today so I thought I'd better catch up on the blog! Cécile started her ninth month of the re-induction phase this week. Everything is still going according to plan, which is good news.

C'est la Sainte Cécile aujourd'hui donc une mise à jour s'impose! Cécile a commencé le neuvième mois de cette phase de ré-induction. Tout se passe bien.

The holidays seem a long time ago now. Cécile rested the first week then we went away the second week: first to see her brother, Ben, studying in Holland. From there we went to Amsterdam and Brugge. Here are a couple of photos:

Luckily this wasn't during the week of corticotherapy so chocolates could be sampled by all!

Les vacances semblent loin derrière nous maintenant. La première semaine Cécile en a profité pour bien se reposer. La deuxième semaine nous sommes partis d'abord aux Pays Bas pour rendre visite à son frère, Ben. On en a profité pour retourner à Amsterdam, histoire de faire un petit tour au Rijksmuseum avant d'aller à Brugges. La semaine de corticoïdes étant passée, tout le monde a pu déguster les chocolats sans hésitation!

The next step is the quarterly lumbar puncture. Then one more re-induction before the Christmas holidays.

Le prochain rendez-vous sera pour la ponction lombaire trimestrielle. Ensuite il restera une ré-induction avant les vacances de Noël.

Last week of RI 7

It's been a while since the last post - maybe this is a good sign - it means there isn't so much happening on the medical front and Cécile is at last able to lead an almost normal life.

Il était grand temps de faire une mise à jour du blog! La distance entre les billets est plutôt une bonne nouvelle - il y a moins de choses à raconter sur le plan médical et Cécile commence enfin à reprendre une vie à peu près normale.

She started a new school in September. This is normal at her age - in the secondary system in France there are two stages at school: 'collège' from 11 to 14 years old then 'lycée' from 15 to 18. This did make things slightly more complicated as it meant explaining Cécile's condition to a certain number of people in the school so they understand why Cécile may have to be absent from time to time. The first few days were very tiring for Cécile - just moving around from one classroom to another and waiting in the queue at the canteen took some getting used to. And she was exhausted after the first sports lesson! But she's taking it all in her stride and although she gets tired she is coping extremely well.

Elle a fait sa rentrée au lycée en septembre. Le changement d'établissement nous a obligé a mettre la nouvelle équipe enseignante au courant de la condition de Cécile afin de bien gérer ses absences et garder un œil sur elle. Les premiers jours étaient très fatigants - il lui a fallu un peu de temps pour s'habituer physiquement aux changements de salle toutes les heures et la queue à la cantine ... Et le premier cours de EPS l'a épuisée! Mais elle s'adapte et fait preuve de beaucoup de volonté, comme d'habitude.
 
She has started playing the piano again and also having ballet lessons - just once a week for the moment while she adapts to the new rhythm.

Elle a repris les cours de piano et reprend tranquillement la danse - un cours par semaine pendant qu'elle s'habitue à son nouveau rythme.
 
From a medical point of view she has had her quarterly lumbar puncture, her seventh reinduction and a trip to the Debré hospital to see her hematologist there. All this is now more complicated to plan, trying to book appointments so Cécile's school timetable is not disrupted too much - so far she's only missed a handful of classes and of course, always catches up immediately!

D'un point de vue médical elle a eu sa ponction lombaire trimestrielle, sa 7ème ré-induction et un rendez-vous avec son médecin référent à l'Hôpital Debré. Tout devient plus compliqué au niveau de la prise des rendez-vous afin d'éviter trop d'absences au lycée. Pour l'instant Cécile a manqué une poignée de cours - tous rattrapés aussitôt!
 
The school holidays start at the end of next week and the two-week break will give Cécile the chance to rest and hopefully fit in a few more trips out, like last Sunday when we went to the third floor of the Eiffel Tower - the first time she's been able to get right to the top!

Les vacances scolaires commencent à la fin de la semaine prochaine. Cécile pourra en profiter pour bien se reposer et faire d'autres sorties, comme celle de dimanche dernier quand elle a enfin pu atteindre le troisième étage de la Tour Eiffel!

RI 6 - Week One

Cécile started her 6th Reinduction yesterday with her intravenous chemo in hospital. Her blood count was OK apart from one of the liver enzymes but this can be a side effect of the chemotherapy.

Cécile a commencé sa 6ème Ré-induction hier après une demi-journée à l'hôpital pour sa chimio par intraveineuse. Son bilan était correct à part le taux des enzymes du foie qui était un peu élevé, un des effets secondaires de la chimiothérapie.

Le Clos Lucé

This past month has been relatively uneventful from a medical point of view. Cécile still has problems with her knees but the x-ray taken last month showed there were no signs of osteonecrosis, a possible side-effect from the corticotherapy. The doctors don't seem particularly concerned. It has meant that any walks have had to be adapted to Cécile's state of fatigue. We're hoping that the pain will start to subside as it is rather a handicap for Cécile. She has also been having more nausea recently so she's been subscribed some medication to help relieve the symptoms.

Le mois d'août a été relativement calme sur le plan médical. Cécile a toujours des problèmes de genoux mais la radio prise le mois dernier ne montre aucun signe d'ostéonécrose, un des effets secondaires de la corticothérapie. Les médecins ne sont pas spécialement inquiets mais Cécile souffre quand même et nos balades doivent être adaptées à son niveau de fatigue. Depuis quelques semaines elle souffre à nouveau de nausées donc le médecin a prescrit un médicament pour soulager les symptômes.  

Le Vieux Lyon

Cécile has been able to profit from the month of August with various outings and short breaks. We've been to Amboise for a couple of days then down to Lyon to visit her brother who moved there in June. We also went to Strasbourg for a few days, visiting the city and environs and sampling the local produce!

Cécile a profité de ces dernières semaines pour faire quelques sorties. Nous sommes allées à Amboise et ensuite à Lyon pour rendre visite à son frère qui y habite depuis le mois de juin. Nous avons passé quelques jours à Strasbourg pour visiter la ville et les environs et goûter la choucroute et les tartes aux myrtilles!

The school holidays are nearly over and this last week will be spent catching up with friends and getting things ready for going back to school at the beginning of September. Next post in a month's time.

Les vacances scolaires touchent à leur fin et cette dernière semaine sera consacrée aux retrouvailles avec les copines et à la préparation des affaires pour la rentrée au lycée. Prochain billet dans un mois.

RI 5 - Week one

The Doppler scan showed that no damage has been caused to Cécile's liver. With this good news we set off on holiday: first a week in Annecy and then a week in Barfleur. Cécile was able to do lots of activites: cycling, swimming, canoeing, walking ... it's done her a world of good and she looks a picture of health.

L'échographie Doppler montrait que le foie de Cécile n'a pas été abîmé par la chimio. Une bonne nouvelle! Nous sommes partis en vacances, soulagés, et Cécile a pu en profiter pour faire plein d'activités: du vélo, du kayak, de la rando, de la baignade ... cela lui a fait beaucoup de bien et elle a très bonne mine.

After two weeks on holiday we had to get back for Cécile's fifth reinduction. Her blood count was ok and she had her chemo yesterday. It was difficult having to return to hospital after the holidays but the nurses, as usual, were cheerful and positive and the session was soon over. Next post in a month's time!

Après deux semaines de vacances il fallait rentrer chez nous pour la réinduction n° 5. Le bilan sanguin était correct et Cécile a eu sa chimio par intra-veineuse hier. Le retour à l'hôpital était difficile après les vacances mais les infirmières étaient souriantes et aimables, comme d'habitude, et la séance était vite finie. Rendez-vous dans un mois pour le prochain billet!

RI 4 - week one

After satisfactory results for her blood test, Cécile had her intraveinous chemo on Monday (29th June) marking the beginning of the Fourth Reinduction. Quick reminder: at the start of each reinduction Cécile also has a week of corticosteroids which unfortunately means the salt-free, sugar-free diet.

Les résultats du bilan lundi matin (29 juin) étaient satisfaisants et Cécile a pu avoir sa chimio par intra-veineuse, ainsi commençant la 4ème Réinduction. Petit rappel: chaque début de réinduction est accompagné d'une semaine sous corticoïdes ce qui veut dire un régime sans sel et sans sucre.

On Tuesday she had an appointment with her hematologist at the Debré hospital. Her doctor was satisfied with the clinical examination but just seemed concerned about the level of platelets which remains rather low. She has asked us to take Cécile for a hepatic doppler scan to make sure that the chemo isn't causing damage to Cécile's liver. We've got an appointment at the end of next week.

Cécile avait rendez-vous avec son médecin référent à Debré mardi après-midi. Le médecin était satisfaite avec l'examen clinique mais comme les plaquettes de Cécile ont du mal à remonter à un niveau satisfaisant, elle a prescrit une échographie doppler hépatique avant de contrôler l'effet de la chimio sur le foie de Cécile. Nous avons rendez-vous à la fin de la semaine prochaine.

Otherwise all is well. Cécile is relaxing now and getting into holiday mode. Today we went to Compiègne to visit the museum where you can see the the railway carriage in which the Armistice was signed at the end of WWI.

Sinon tout va bien. Cécile commence à se reposer et adopte petit à petit le mode 'vacances'. Aujourd-hui nous avons visité le Musée de l'Armistice où l'on peut voir le wagon dans lequel l'armistice de 1918 a été signé.

RI 3 - First picnic of the year in Fontainebleau!

Cécile was able to have her lumbar puncture last Monday (22nd June) as she hadn't had any fever since Friday evening. It was a long day - we arrived at the hospital at 9h30 and had to wait until gone 3pm for the lumbar puncture. Cécile was apprehensive about the side effects (headaches and sickness) so the doctor recommended drinking plenty of coca cola over the first 24 hours after the injection. We're not sure if that was the reason but Cécile didn't suffer at all afterwards.

Cécile a eu sa ponction lombaire lundi dernier (22 juin) car cela faisait plus de 48 heures qu'elle n'avait plus de fièvre. La journée était longue - nous sommes arrivées à l'hôpital à 9h30 et la ponction lombaire a eu lieu après 15 heures. Cécile appréhendait des effets secondaires - maux de tête et vomissements - mais peut-être grâce à la consigne de boire plusieurs cannettes de coca cola pendant les premières 24 heures elle n'a pas eu de séquelles.

The rest of the week was spent revising for the exams Cécile was taking last Thursday and Friday. Then finally she was able to relax! We decided to take the opportunity to go to Fontainebleau for our first picnic there this year. The weather was lovely and it was nice to benefit from the shade of the forest.

Cécile a eu deux jours pour réviser avant de passer le DNB jeudi et vendredi. Enfin elle pourra se détendre! Nous avons profité du beau temps ce week-end pour faire notre premier pique-nique à Fontainebleau. Tout le monde a apprécié l'ombre dans la fôret!

RI 3 - Father's Day

Even though Cécile wasn't going to have her lumbar puncture on Friday (19th June), the doctor decided she wanted to see Cécile anyway to try to understand where the fever was coming from. Cécile wasn't happy about yet another trip to hospital (third in one week) but it was worth going. While the doctor was examining Cécile, she mentioned that her throat hurt a bit when she drank. The doctor took a look and it turns out Cécile has tonsilitis. This would explain the fever she's had since last Monday. It still means the treatment is on hold as long as the fever persists but at least we have an explanation, which is quite reassuring.

Même si la ponction lombaire n'allait pas avoir lieu vendredi (19 juin), le médecin a tout de même souhaité voir Cécile afin d'essayer de comprendre d'où venait la fièvre. Cécile n'avait pas envie de retourner à l'hôpital pour la troisième fois cette semaine mais finalement ça valait le déplacement. Pendant que le médecin auscultait Cécile elle lui a parlé d'une petite gêne lorsqu'elle buvait. Le médecin a jeté un œil ... et Cécile a une angine blanche. Ceci expliquerait donc la fièvre qui dure depuis lundi dernier. Le traitement est toujours en attente pendant que Cécile a de la fièvre mais au moins nous comprenons d'où elle vient, ce qui nous rassure.

Cécile's specialist in Paris phoned to say they would be doing a different blood analysis next Monday to see if Cécile has a particularly low count of lymphocytes which could explain why she has picked up two viral infections in four weeks. If this is the case Cécile will have to have transfusions of white blood cells. We'll find out next week if that's going to be necessary.

Le médecin référent de Cécile a téléphoné de Paris pour dire qu'il y aurait d'autres analyses faites avec le bilan sanguin demain (lundi) afin de contrôler le nombre de lymphocytes. Si le taux est trop bas cela pourrait expliquer pourquoi Cécile a attrapé deux virus en l'espace de quatre semaines. Si c'est le cas elle aura peut-être besoin de transfusions de globules blancs. On saura la semaine prochaine.

Cécile's temperature was still high on Friday night but since then her temperature hasn't been up to 38° so hopefully the treatment will resume tomorrow.

Cécile avait toujours de la fièvre vendredi soir mais depuis elle est restée en dessous de 38°. Normalement le traitement pourra reprendre demain.

Meanwhile, today was Father's Day! As the boys are both away Cécile was in charge - as you can see on the photos there were smiles all round!

En attendant, aujourd'hui c'était la Fête des Pères! Comme les garçons ne sont plus à la maison Cécile a tout organisé - vous pourrez constater sur les photos qu'elle a retrouvé le sourire!

RI 3 - still no lumbar puncture

Cécile still hasn't been able to have her lumbar puncture. She has a swinging fever: a normal temperature in the morning then slowly rising during the day. As long as she is still reaching 38° or more they won't do the lumbar puncture.
Cécile saw the doctor again on Wednesday as she was at the hospital anyway for an adolescent support group. Another blood test was done, the results for which were satisfactory. But still no explanation for the high temperatures Cécile is reaching in the evenings. The doctor put Cécile on another course of antibiotics just in case.
Her temperature isn't getting so high - she peaked at 39,6° on Tuesday evening but last night the maximum was 38,6°. Let's hope the fever subsides over the weekend so the lumbar puncture can be done on Monday.
All of this isn't helping Cécile with her revision for written exams at the end of next week. The fever makes her tired and she's not able to concentrate for any length of time. Maybe the antibiotics will have a positive effect ...

Cécile n'a toujours pas eu sa ponction lombaire. Elle fait de la fièvre intermittente: température normale le matin qui grimpe tranquillement dans la journée. Mardi soir elle a atteint 39,6°. Tant qu'elle aura de la fièvre la ponction lombaire n'aura pas lieu.
Cécile a revu le médecin mercredi car elle avait rendez-vous pour un groupe de paroles pour adolescents. On lui a fait une prise de sang et les résultats étaient satisfaisants mais toujours pas d'explication pour la fièvre. Le médecin a prescrit des antibiotiques au cas où ...
Espérons que la fièvre disparaitra tranquillement pendant le week-end pour que la ponction lombaire puisse être faite lundi.
Cécile est de plus en plus fatiguée avec la fièvre, ce qui n'arrange pas les choses pour ses révisions. En effet, elle passe le brevet des collèges à la fin de la semaine prochaine. Peut-être les antibiotiques feront de l'effet ...

Reinduction Number 3 - Still not quite plain sailing ...

So - as mentioned in the previous post, Cécile went back to hospital on the 10th June for a check up and blood test. It was all very quick - for a change! Cécile went to school in the morning and we arrived at hospital just after 1pm. The blood test was done almost straight away and the doctor came to see Cécile with the results just before 2.30pm. The neutrophils (white blood cells) were well up but the platelets were still not at a satisfactory level so the doctor is leaving Cécile on the lower doses of oral chemo she's been taking since the 3rd June until next week at least.

By the weekend Cécile was showing signs of fatigue. This was probably quite normal as she had just had her first full week of school going morning and afternoon and with homework to boot! Not to mention the hot weather ... On Saturday evening we had our annual street party and all the neighbours were pleased to see Cécile looking good and happy to be able to attend.

On Monday she was back to hospital after a morning in school for her first quarterly lumbar puncture. She had been fine on arrival but suddenly developped a temperature which rose slowly while she waited for the blood test results. The doctor had to cancel the lumbar puncture as any chemo is contra-indicated in case of fever. Her fever slowly rose during the day but Cécile wasn't in any discomfort so she was allowed to go home.

Tuesday morning (16th) the temperature has gone down but Cécile is very tired. She shall stay at home today and hopefully be on form for Thursday when she must return to hospital for the lumbar puncture, all being well.

Cécile est donc retournée à l'hôpital mercredi 10 juin après les cours du matin pour un bilan sanguin. Tout s'est bien passé - et pour une fois c'était plutôt rapide! Elle est arrivée vers 13 heures et le médecin est passé la voir juste avant 14h30 avec les résultats - un record! Les PNN sont en nette augmentation mais les plaquettes trainent encore. Du coup le médecin a décidé de laisser les doses de chimio orale au niveau bas en attendant un nouveau bilan.

A la fin de la semaine la fatigue commençait à se sentir, pas vraiment étonnant vue que Cécile venait de faire sa première semaine complète de cours matin et après-midi et en plus des devoirs le soir. Et il a commencé à faire chaud en fin de semaine ... Samedi soir nous avons participé à notre annuelle fête des voisins dans notre rue. Les voisins étaient contents de voir Cécile en forme et heureuse d'être présente à la fête.

Lundi il fallait retourner à l'hôpital après les cours du matin pour la première des ponctions lombaires trimestrielles. Cécile allait très bien mais d'un seul coup sa température a commencé à monter. Comme la fièvre ne baissait pas le médecin a été obligé d'annuler la ponction lombaire car la chimio est contre-indiquée en cas de fièvre. Cécile était autorisée à rentrer à la maison à condition qu'elle se repose.

Mardi matin Cécile n'avait plus de fièvre mais elle est toujours très fatiguée. Elle reste à la maison aujourd'hui. On retourne à l'hôpital jeudi si il n'y a pas d'autres périodes de fièvre pour faire la ponction lombaire.

Back to school again

The good news is that the blood test showed that the number of white blood cells had increased sufficiently for Cécile to have her intravenous chemo and to return to school, which she did on Wednesday 3rd June. She was pleased to be back to normal again.

Unfortunately the number of platelets has gone down so we must return to hospital next Wednesday, 10th June, for another blood test to check the levels.

Friday was a long, hot day with seven hours of class and rapidly rising temperatures - luckily Cécile was able to cool off in the pool - well covered in high protection sunscreen as Cécile's skin is now very sensitive to sunlight due to the chemo she has had.

La bonne nouvelle: Cécile est sortie d'aplasie! Le bilan de mercredi montrait que les globules blancs ont bien remonté et Cécile a pu reprendre son traitement avec une chimio en intraveineuse et surtout elle était autorisée à retourner au collège! Elle était contente de retrouver une vie normale.

Malheureusement les plaquettes sont en baisse. Du coup, il faut retourner à l'hôpital mercredi 10 juin pour un nouveau bilan.

La journée de vendredi était longue et chaude: 7 heures de cours et des températures extérieures en nette hausse. Heureusement Cécile a pu se rafraichir dans la piscine ... avec une crème haute protection car sa peau est devenue très sensible à cause de la chimio.

30th May

The blood test yesterday showed Cécile is still neutropenic, ie. she doesn't have many white blood cells. So she can't go back to school yet, she couldn't have her intravenous chemo and the oral chemo is still suspended. Back to hospital on Tuesday 2nd June for another blood test.

Le bilan d'hier montre que Cécile est toujours en aplasie. Elle ne peut pas retourner à l'école pour l'instant, sa chimio orale est toujours en attente et elle n'a pas pu avoir sa chimio en intraveineuse. On retourne à l'hôpital mardi 2 juin pour un nouveau bilan.

Unfortunately the results of the blood test today showed that Cécile still doesn't have enough white blood cells so she must stay off school. Back to hospital on Friday for another blood test and maybe her intravenous chemo (Reinduction number 3) if the results are OK.

Malheureusement les résultats du bilan d'aujourd'hui montrent que Cécile est toujours en aplasie et ne peut pas encore retourner au collège. On retourne à l'hôpital vendredi pour un nouveau bilan et peut-être sa chimio en intraveineuse (Ré-induction n° 3) si les résultats la permettent.

Good days and bad days

Cécile's birthday last weekend went very well. The weather was great and Cécile really enjoyed her day in the company of her family and some close friends.

Cécile a passé une très bonne journée dimanche dernier. Il a fait beau et Cécile a bien profité de son anniversaire entourée de famille et amis.

Unfortunately things haven't been so good since. Cécile had a high temperature and had to go to hospital on Tuesday afternoon for a check-up and a blood test. The results of the blood test showed a slightly high CRP level - an indication that she had an infection. The doctors decided she should have a chest x-ray, which was clear. After discussion with the doctors at Debré, Cécile was allowed home with a week's course of antibiotics and an appointment to have another blood test on Friday.
The blood test on Friday showed that Cécile was once more neutropenic, ie very low on defenses. Her daily and weekly oral chemo has been stopped for the moment and she must return to hospital next Wednesday for another blood test. The hardest part for Cécile is that she can't go to school. It's rather a blow having only recently been able to return to school. This maintenance phase isn't plain sailing.

Malheureusement depuis les choses se sont gâtées. Elle a fait de la fièvre et mardi il fallait aller à l'hôpital pour un bilan sanguin. Les résultats n'étaient pas inquiétants à part un taux de CRP (indicateur d'infection) un peu élevé. Du coup il fallait passer une radio des poumons. La radio était ok donc après discussion avec les médecins de Debré, Cécile a été autorisée à rentrer à la maison avec des antibiotiques et un rendez-vous à l'hôpital vendredi pour un bilan de contrôle.
Le bilan de contrôle a donc été fait hier et a montré que malheureusement Cécile est en aplasie. Son traitement de chimio est arrêté en attendant que les taux retrouvent un niveau convenable. Le pire pour Cécile est qu'elle ne peut pas aller au collège. C'est très dur d'être à nouveau obligée de rester à la maison après avoir repris les cours il y a si peu de temps. Elle doit retourner à l'hôpital mercredi prochain pour un nouveau bilan.

After a hard day at school, what better way to relax than taking a dip in the pool! Let's hope the weather stays fine to celebrate Cécile's birthday at the end of the week ...

Quelques longueurs après les cours - rien de tel pour se détendre! Espérons que le soleil sera au rendez-vous à la fin de la semaine pour fêter l'anniversaire de Cécile ...

First week of school

It's Sunday evening and Cécile is getting into the routine of going to bed at a reasonable time. Her first week back at school went well. On Monday morning the understandable apprehension soon dissipated when she arrived at school and her friends came to greet her. She's doing mornings only for the first two weeks so as not to get overtired and she felt the time went quickly. However, by the end of the week she was noticeably weary and spent the bank holiday (VE Day) reading a book. We've had a steady weekend, initial plans for walks and picnics were abandoned in order to catch up on homework and prepare for tests next week. Like a normal school girl.

Regarding her medication, she finished her week of corticosteroids on Thursday and celebrated with sausage, bacon and eggs! Not the healthiest of meals but so good for the morale!


On est dimanche soir et comme toute bonne élève, Cécile prévoit de ne pas se coucher trop tard! Cette première semaine de cours s'est bien passée. Ses angoisses se sont vite dissipées en arrivant devant le collège lundi matin avec toutes ses amies qui sont venues l'embrasser. Pour l'instant elle ne reste au collège que le matin afin de privilégier une reprise en douceur. Cécile a trouvé que le temps a passé très vite. Cependant, à la fin de la semaine elle était visiblement fatiguée et a profité du jour férié pour ne rien faire à part lire un roman. Nous avons passé un week-end tranquille. Le pique-nique et la balade initialement prévus ont été annulés afin de rattraper les devoirs et préparer les contrôles de la semaine à venir.

En ce qui concerne son traitement, elle a fini la semaine de corticoïdes pour le ce mois-ci et a fêté la fin des restrictions alimentaires avec un petit déjeuner anglais: saucisses, bacon et œuf au plat! Pas très diététique mais tellement bon pour le moral!

Back to school! La rentrée!

Cécile has had a quiet week catching up on homework and tests. The physiotherapist has been a few times and the exercises are getting more and more strenuous! She had one day in hospital, on Thursday, for her second reinduction (RI2).

Reminder: there will be 12 reinductions, one every four weeks. Cécile has to go to hospital and once the blood test has confirmed she has the necessary conditions, she has an intravenous injection of Vindesine.

On Thursday evening Cécile had a high temperature. She had had the same reaction one month ago (see post for the 4th April) but as she no longer has the catheter, we were able to keep her at home. A dose of Dolipran brought her temperature back down to normal and on Friday morning she was OK.

She's now getting ready to go back to school tomorrow morning. It will be a very emotional time for her after so many months away from school but we are confident that her friends and her teachers will make sure that everything goes as well as possible. But in the meantime her stress levels are going up ...


Cécile a passé une semaine tranquille avec des séances de kiné (qui deviennent de plus en plus toniques!), du rattrapage de devoirs et de contrôles et une journée à l'hôpital jeudi pour la deuxième ré-induction (RI2).

Petit rappel: il y aura 12 ré-inductions, une toutes les quatre semaines. Cécile doit se rendre à l'hôpital et après un bilan sanguin pour vérifier qu'elle a les conditions nécessaires, elle reçoit une dose de Vindésine en intraveineuse.

Le soir-même elle a fait une forte poussée de fièvre. Elle a eu la même réaction il y a un mois (cf. page du 5 avril) mais comme elle n'a plus le cathéter, nous pouvons gérer la fièvre à la maison. Une dose de Dolipran a suffi pour faire baisser la fièvre et vendredi matin Cécile allait mieux.

Elle se prépare pour le retour en cours demain. Un moment d'émotion très forte, sans doute, après tous ces mois d'absence mais nous savons que nous pouvons compter sur ses amis et ses enseignants pour que la reprise se passe du mieux possible. Mais en attendant le stress monte ...

As the catheter hadn't been removed on the 16th April as initally planned we had to go to Sud Francilien on Thursday to have the dressing changed - hopefully this would be the last time.

Comme le cathéter n'a finalement pas été enlevé le 16 avril, il fallait faire un tour au Sud Francilien pour changer le pansement - en espérant que cette fois-ci serait la dernière ...

Because we were going to Barfleur, Cécile's hematologist had to contact the hospital in Cherbourg just in case we needed to take her there. The doctor told us we would need to have a blood test done on Tuesday 21st to check Cécile's blood count before the operation.

Le médecin référent de Cécile a contacté le service pédiatrique de l'hôpital de Cherbourg en cas de problème pendant notre séjour à Barfleur. Elle nous a donné une ordonnance pour faire faire un prise de sang mardi 21 afin de contrôler le bilan sanguin de Cécile avant l'opération.

On Saturday we set off for Barfleur: François, Cécile, Benjamin, myself and the dogs. The weather was good and expected to stay so. As soon as we arrived we went down onto the beach - rather chilly, very windy but beautifully sunny!

Nous avons pris la route pour Barfleur samedi: François, Cécile, Benjamin, moi et les chiens. Il faisait beau et les prévisions étaient bonnes pour le week-end. Dès notre arrivée nous avons fait un tour sur la plage - un peu frisquet, très venteux mais magnifiquement ensoleillé!

Cécile was in good form and we managed to fit a lot in during our long weekend. Walks on the beach, of course, with Benjamin in charge of trying to exhaust Hopper ... not an easy task!

Cécile était en forme et nous avons réussi à bien remplir nos journées. Des balades sur la plage, bien sûr, pendant lesquelles Benjamin essayait (en vain!) de fatiguer Hopper!

 

We went to Sainte Mère Eglise to visit the Airborne Museum and take a quick look at Utah Beach.

Nous sommes allés à Sainte Mère Eglise pour visiter le Musée Airborne et faire un tour à Utah Beach.

A trip we have done many times before, we took the little boat across to Tatihou and walked round the island.

Comme souvent dans le passé, nous avons pris le petit bateau pour nous rendre sur l'Ile de Tatihou.

As we had to go all the way to Cherbourg to have the blood test, we had a look round the shops, went on a boat trip round the harbour and then stopped off at the beautiful Cap Lévi on the way home.

Comme il fallait aller jusqu'à Cherbourg pour faire la prise de sang nous en avons profité pour faire les magasins, faire un tour de la rade en bateau et au retour faire une halte au Cap Lévi, toujours aussi beau.

Even after all this walking, Cécile still managed a little race on the beach with her father:

The results of the blood test were satisfactory so we left Barfleur on Wednesday in order to be home in time for the operation on Thursday morning.

Les résultats du bilan étaient bons et nous avons quitté Barfleur mercredi afin d'être rentrés pour l'opération jeudi matin.

So finally, after ten months, the catheter was to be removed. The operation went well. After about an hour in the recovery room Cécile was allowed up. She felt a bit weak and unfortunately the effect of the anesthetic made her sick a couple of times. The wound was rather sore but this was to be expected. For the rest of the day after the op she didn't feel great, probably due to the anesthetic, the morphine and the emotion. On Friday she started feeling a lot better even if the wound is still rather sore. The stitches should dissolve after 8 or 10 days.Very soon now Cécile will be able to shower again normally, one of life's little pleasures we tend to take for granted!

Enfin, au bout de dix mois, on allait enlever le cathéter. L'opération s'est bien passée. Après une heure en salle de réveil Cécile avait le droit de se lever. Elle se sentait un peu faible et a été prise de vomissements, probablement suite à l'anesthésie et la morphine qui ont été administrées. Comme on pouvait s'y attendre, le site de l'ablation était très tendre. Le cumul nausée, douleur, émotion et anesthésie a fait que Cécile n'était pas en grande forme pour le reste de la journée. Dès le lendemain elle se sentait mieux, même si la plaie reste un peu douloureuse. Les points de suture vont se dissoudre d'ici 8 à 10 jours. Bientôt les douches à rallonge!

Catheter ...

We returned home from Saint Jorioz on Monday ready for two appointments / meetings on Tuesday:

In the morning Cécile accompanied me to her school for a meeting with the headmistress, some of Cécile's teachers and a children's cancer nurse to discuss how best to organize Cécile's return to school. The school holidays end on the 3rd May so the idea is that Cécile will return to school on the 4th, but for mornings only initially. We'll see how she copes and decide after the first two weeks whether she can then go full time. The teachers had lots of helpful suggestions about break-time and general logistics for getting from one class to another to make things as easy as possible for Cécile. It was reassuring to see how concerned everyone is about Cécile's wellbeing. She managed to see a couple of her friends in the playground - having been back in the school after all these months will probably make her first day back in May easier to cope with.

Then in the afternoon we had an appointment with her hematologist in Paris. A blood test was done to check the white blood cells in view of having the catheter removed on Thursday. The doctor said she would phone in the evening if there was a problem.

Around 7pm the phone rang and the doctor announced the disappointing news that Cécile had too few white blood cells for the surgeon to take the risk of removing the catheter due to an increased probability of infection. It was rather a blow as Cécile - and the rest of us - had been counting the days to this operation which really marks the end of the heavy chemo.

After some negotiation, the operation has been reprogrammed for next Thursday, 23rd April (Saint George's Day ...) so fingers crossed that the blood count will be better by then. The doctor has reduced the doses of oral chemo in order to relieve slightly the pressure on the bone marrow.

The good news is that we're going away again this weekend - to Barfleur at last! The weather forecast looks reasonable so we're looking forward to a few sunny days by the sea.

Nous sommes rentrés de Saint Jorioz lundi afin d'être prêtes pour deux rendez-vous / réunions mardi:

Mardi matin Cécile m'a accompagnée à son collège pour une réunion avec la principale, certains enseignants et l'infirmière coordinatrice du Rifhop (réseau Ile de France d'hématologie et oncologie pédiatrique) afin de préparer le retour en classe de Cécile. Les vacances scolaires se terminent le 3 mai et Cécile va reprendre le 4 mai, à mi-temps. Après les quinze premiers jours on verra comment cela se passe pour voir si éventuellement elle pourra par la suite reprendre à temps complet. Les enseignants ont fait quelques suggestions d'un point de vue logistique pour les déplacements de Cécile et les récréations - c'est très rassurant de voir tout le monde se préoccuper du bienêtre de Cécile. Elle a vu quelques copines dans la cour - je pense que ce premier retour au collège après tant de mois facilitera le premier jour de reprise en mai.

L'après-midi nous avions rendez-vous avec le médecin référent à Debré. Une prise de sang a été faite afin de vérifier les globules blancs avant l'opération pour enlever le cathéter prévue pour jeudi. Le médecin devait appeler le soir si il y avait un problème.

Vers 19h00 le téléphone a sonné. Grosse déception: Cécile n'avait pas assez de PNN pour que le chirurgien accepte de faire l'intervention. Cécile la première mais nous aussi étions très déçus - on attendait ce jour avec impatience et de voir l'échéance s'éloignait encore était dur à accepter.

Après discussion, l'opération a été reprogrammée pour jeudi prochain, le 23 avril. On croise les doigts pour que cette date soit la bonne. Le médecin a baissé les doses de chimio orale afin de réduire légèrement la pression sur la moelle épinière.

La bonne nouvelle est que nous allons repartir ce week-end - à Barfleur enfin! Les prévisions météo sont corrects donc nous espérons passer quelques journées ensoleillées au bord de la mer.

Weekend in Saint Jorioz!

As they say in French: 'Après l'effort le réconfort' ... and it was certainly the case this week!

On Tuesday Cécile had appointments with her hematologist and the anesthetist at the Debré hospital. Unfortunately her hematologist had to cancel but we went anyway to see the anesthetist. Of course, it was complicated because Cécile's dossier was not in the right place because of the cancelled appointment but we managed to get things sorted! A lot of hassle for a very short appointment and a huge traffic jam on the motorway - that's Paris!

On Wednesday and Thursday Cécile did some mock exams at home - at the end of June she will be sitting the Diplôme National du Brevet - it's similar to GCSEs but taken a year earlier. She had been rather stressed but they seemed to go OK.

Also on Wednesday we went to Sud Francilien for a blood test and check up - and to discuss with the doctor the possibility of going to Saint Jorioz for the weekend. The doctor gave us the all clear so on Thursday afternoon, Cécile, Nicolas and I set off with the dogs for Cécile's first weekend away!

It takes nearly six hours so Cécile was tired on arrival and the first night was rather short.

On Friday, a good friend of mine came down from her home on the mountainside with her two white Swiss shepherd dogs and we had a lovely day together, walking the dogs and catching up. Cécile was tired but happy in the evening.

On Saturday, her aunt Lucy who lives in Lausanne came for lunch with her youngest, Anna, and again, we had a good time, chatting, walking the dogs by the lake and the two cousins played together.











And today Cécile, Nicolas and I took the dogs out for a different walk - not quite in the mountains - Cécile isn't up to real hiking yet - but a lovely walk in the pure mountain air where the dogs could run around.

In the afternoon we were invited for tea with our new neighbours, a charming couple with two lovely daughters. After a couple of hours chatting and getting to know each other, they offered to take us for a trip round the lake on their motor boat! What a wonderful way to end the weekend!







Après l'effort le réconfort ...

Mardi dernier Cécile avait des rendez-vous avec son médecin référent et l’anesthésiste  à l'hôpital Debré. Malheureusement le médecin a dû annuler son rdv mais nous y sommes allées quand même pour voir l'anesthésiste. Bien sûr, le dossier ne se trouvait pas au bon endroit mais après quelques coups de fil on a pu voir l'anesthésiste ... pendant dix minutes après avoir passé plus d'une heure et demi dans les embouteillages parisiens ...

Mercredi et jeudi Cécile passait les examens blancs à la maison pour le brevet qu'elle passera fin juin. Elle était assez stressée mais tout s'est bien passé.

Mercredi nous avons également fait un aller-retour au Sud Francilien pour un bilan et pour discuter avec le médecin de la possibilité d'emmener Cécile à Saint Jorioz ce week-end. Le médecin nous a donné le feu vert donc jeudi après-midi Cécile, Nicolas et moi sommes partis avec les chiens pour la première grande sortie de Cécile!

Le trajet dure environ six heures et Cécile était fatiguée en arrivant et la première nuit était plutôt courte.

Vendredi, une bonne amie est descendue de la montagne avec ses deux bergers blancs suisses et nous avons passé une bonne journée ensemble à promener tous les chiens et à rattraper le retard depuis la dernière fois que nous nous sommes vues. Le soir Cécile était fatiguée mais heureuse.


Samedi, sa tante Lucy qui habite Lausanne est venue avec sa petite dernière, Anna, et à nouveau nous avons passé une très belle journée à bavarder entre filles! Nous avons promené les chiens le long du lac et les deux cousines se sont bien amusées ensemble.









Et aujourd'hui, Cécile, Nicolas et moi avons promené les chiens dans l'arrière-pays - pas tout à fait dans les montagnes car Cécile n'est pas encore prête pour une vraie randonnée. Nous avons trouvé un petit chemin tranquille avec une jolie vue sur le lac et les montagnes où les chiens avaient le droit de s'amuser.

Cet après-midi nous étions invités pour prendre le thé chez nos nouveaux voisins, un couple très accueillant avec leurs deux filles charmantes. Après deux heures passées à faire connaissance ils ont proposé de nous emmener faire un tour du lac dans leur bateau à moteur! Quelle fin merveilleuse pour notre week-end!

Easter Sunday

The week hasn't been as tranquil as we'd been hoping.

The nurse came on Monday morning to take a blood sample. The recent events have made Cécile quite anxious about her results every time she has a blood test and she was relieved when we got the results early afternoon: 984 neutrophiles and 46000 platelets. To celebrate we went clothes shopping!

Then on Tuesday Cécile had some problems with the catheter, possibly due to the valve which had been changed on Monday. The doctor suggested calling into hospital on Wednesday to have it checked over. While we were in on Wednesday morning the nurse did a blood test and the results were good enough for Cécile to start the two-year maintenance phase: over 1000 neutrophiles and 99000 platelets (required numbers: 1000 neutrophiles and 100 000 platelets). This was rather unexpected. The doctor said she'd try to get hold of the chemo and Cécile could start the treatment that same day! While we were waiting for the chemo to arrive we went shopping and had some lunch - the second time Cécile has been to a restaurant since her treatment started. We were feeling quite euphoric - this was it: Cécile had finished the intensive treatment and would soon be able to lead a fairly normal life.

Unfortunately the euphoria was short-lived. In the evening Cécile had a high temperature and her doctor, who happened to be on call that night, said it would be advisable for Cécile to be kept under observation in hospital overnight. This was rather unexpected, especially as every other time she has had to go to hospital with a temperature she has been neutropenic, which wasn't the case this time.

So another night in hospital. We'd hoped she'd be allowed out the next morning but the doctors were playing it very safe and kept her in until late afternoon after another blood test had been done to check the CRP level which is an indicator of inflammation. All was well so Cécile was home on Thursday evening but rather upset by this latest hospitalization.

So - a quick recap of what this maintenance phase is all about.
For the next two years:
- a daily dose of Purinethol (chemo)
- a weekly dose of Methotrexate (chemo)
- every three months a lumbar puncture with 3 types of chemo.

And at the same time but for the next 12 months:
- a trip to hospital once a month for an intravenous dose of Vindesine (chemo)
- 7 days a month a course of corticosteroids

Today is Easter Sunday. The sun is shining and we've had an egg hunt in the garden, even if Cécile can't eat any eggs until next Thursday as she is on corticosteroids this week. Roll on Thursday!

La semaine n'a pas été aussi tranquille que prévue.

L'infirmière est passée lundi matin pour faire un bilan sanguin. Depuis les dernières transfusions et hospitalisations Cécile est plutôt anxieuse en attendant les résultats et était très soulagée lorsque nous avons appris en début d'après-midi que tout allait bien! 984 PNN et 46000 plaquettes. Pour fêter ces résultats nous sommes allées faire les magasins!

Et puis mardi Cécile a eu quelques soucis avec son cathéter, peut-être à cause d'une valve qui a été changée lundi. Le médecin a suggéré un petit aller-retour à l'hôpital mercredi matin pour contrôler. En même temps l'infirmière a fait une prise de sang et à notre grande surprise les résultats étaient suffisamment bons pour démarrer la phase d'entretien! Plus que 1000 PNN et 99000 plaquettes (niveau requis: 1000 PNN et 100 000 plaquettes). Le médecin a contacté le service nécessaire pour avoir la chimio et en attendant nous sommes retournées aux magasins avec un déjeuner au restaurant: la deuxième fois que Cécile mange au restaurant depuis le début de son traitement. Nous voulions fêter le début de cette nouvelle phase qui correspond à la fin du traitement intense et le début d'une vie à peu près normale pour Cécile.

Mais l'euphorie n'a pas duré longtemps. Le soir Cécile avait de la fièvre et son médecin référent qui était de garde a dit qu'il serait souhaitable que Cécile soit hospitalisée pour la nuit en surveillance. Nous ne nous y attendions pas du tout car lors des autres hospitalisations en état fiévreux Cécile a toujours été en aplasie, ce qui n'était pas le cas cette fois-ci.

Donc une nuit de plus à l'hôpital. Nous pensions la ramener à la maison le lendemain matin mais les médecins jouaient la prudence et l'ont gardée jusqu'en fin d'après-midi après un deuxième bilan sanguin qui montrait que les taux étaient normaux, surtout le CRP, un indicateur d'inflammation. Cécile était de retour à la maison jeudi soir, un peu bouleversée par cette dernière hospitalisation.

Un petit rappel pour expliquer la phase d'entretien.

Pendant deux ans:
- une dose quotidienne de Purinethol (chimio)
- une dose hebdomadaire de Methotrexate (chimio)
- une ponction lombaire avec 3 chimios tous les trois mois

Et en même temps mais pendant 12 mois:
- une journée à l'hôpital une fois par mois pour un bilan et une dose de Vindesine en intravéneuse
- 7 jours de corticoïdes tous les mois

Aujourd'hui on est Dimanche de Pâques. Il y a du soleil et nous avons fait une chasse aux œufs dans le jardin, même si Cécile ne pourra pas en manger avant jeudi, corticoïdes obligent.

Home again ...

What a week!

The few days spent at the Debré hospital were rather hectic. After the platelets on Friday night, Cécile had a blood transfusion on the Sunday (22nd) morning. We all had a laugh in the afternoon with the nursing auxiliaires who've been looking after Cécile on and off since last June (see photo on previous post).
On Monday we were hoping she'd be allowed home as she no longer had a high temperature but the intern came to say they'd be stopping the antibiotics and keeping Cécile in under surveillance for 24 hours to make sure she was OK. Cécile was obviously disappointed at this news.
Then we heard that she was to be transferred to the Sud Francilien hospital as there were several serious cases arriving at Debré and they needed the beds.
So in the afternoon Cécile was transferred to the other hospital by ambulance. She was rather disappointed they didn't put the emergency lights on!

On arrival she was given another transfusion of platelets (4th) and told the next blood test would be on Wednesday. The nurses from the day hospital came to see her and she settled in to her room.
Unfortunately the results on Wednesday were not good. Cécile had hardly any white blood cells, her platelets were down to 7000 despite the transfusions and her hemoglobin had gone down a lot too. She was really upset at the idea of staying in until the weekend. The doctor reassured us that it can happen at this stage of the treatment: Cécile's bone marrow is exhausted and it is taking time getting going again, and the high temperature she had at the weekend wouldn't have helped things. Because the levels were so low she was given another transfusion of platelets (5th) and of hemoglobin (3rd).
She whiled away the time reading, watching TV and DVDs and doing some school work. We also played some games and sometimes the nurses joined in.
Her doctor didn't seem too optimistic for the results on Saturday but Cécile proved her wrong! This morning her neutrophils were back up to 560 (from 140 on Wednesday), her hemoglobin is at 12.6 and she has 30000 platelets. That's still low but it's a huge improvement on 7000!

So - she's come home and will just need a blood test on Monday to make sure things are going in the right direction and hopefully she'll be able to start the two-year maintenance phase on Thursday. For the moment she's just making the most of being home again.

Quelle semaine!

Les quelques jours passés à Debré étaient bien remplis. Après la transfusion de plaquettes dans la nuit de vendredi à samedi, Cécile a eu droit à une transfusion d'hémoglobine dimanche matin. L'après-midi est passé vite en compagnie des aides-soignantes qui s'occupent de Cécile depuis fin juin (voir photo sur page précédente).
Lundi on espérait un retour à la maison mais l'interne est passée pour dire qu'il fallait la garder 24 heures sous surveillance sans antibiotiques. Cécile était très déçue.
Ensuite on nous a annoncé qu'elle allait être transférée au Sud Francilien! Apparemment il y avait des urgences hématologiques et il fallait libérer des lits. Cécile est donc partie en ambulance pour l'autre hôpital. Le trajet s'est bien passé; elle était simplement déçue qu'ils n'aient pas mis le gyrophare ...


En arrivant au Sud Francilien Cécile a eu une 4ème transfusion de plaquettes. On lui a dit qu'il y aurait un bilan mercredi matin pour voir si elle pouvait sortir ou pas.
Cécile a eu des visites des infirmières qui s'occupent d'elle en hôpital de jour. Et elle commence maintenant à bien connaître les équipes qui travaillent dans le service d'ados. Mais le temps est long lorsque l'on ne peut pas sortir de sa chambre. Elle a passé le temps en lisant, en regardant la télé et des DVDs et aussi un peu de révision pour le brevet ...

Malheureusement le bilan mercredi matin n'était pas très bon. Les PNN étaient à 140, les plaquettes à 7000 et l'hémoglobine à 8. Dans la journée elle a eu sa 5ème transfusion de plaquettes et la 3ème transfusion d'hémoglobine. Le médecin nous a rassuré que ce n'était pas inhabituel à ce stade du traitement que la moelle ait du mal à redémarrer, surtout si en plus Cécile était en aplasie fébrile. C'est embêtant mais pas inquiétant. Il faut simplement s'armer de beaucoup de patience.

Son médecin ne semblait pas trop optimiste pour une sortie samedi. Mais Cécile a prouvé le contraire! Ce matin elle avait 560 PNN, 12,6g d'hémoglobine et 30000 plaquettes. C'est toujours un peu bas mais nettement mieux que 7000!

Donc - voilà Cécile de retour à la maison! On fera un bilan lundi pour voir où elle en est et si tout se passe bien, jeudi elle va démarrer la phase d'entretien (qui dure deux ans). Pour l'instant elle profite de sa liberté!

Back to hospital

Another difficult week. The nurse came to take blood samples on Monday morning and in the afternoon the hospital phoned to say we had to take Cécile in for a transfusion of hemoglobin and platelets. She had only 6g of hemoglobin and 20 000 platelets (normal levels are between 150 000 and 400 000). She ended up spending the night in hospital because it took a long time for the blood to arrive and then the transfusion itself takes at least four hours ... She came home on Tuesday morning and seemed on reasonable form.
On Thursday she had to go back to hospital for a check up and the blood test revealed that despite the transfusion on Monday, she only had 10 000 platelets so she had to stay in and have another transfusion.
On Friday she seemed rather tired but we put it down to the fact she'd had a couple of lessons and a session with the physio. But when she took her temperature in the evening it was up to 38.8° and as she is neutropenic (less than 500 neutrophiles), we had to take her to hospital. But the Sud Francilien hospital didn't have any room so we had to go in to Paris, to the Debré hospital. It took a while to get sorted - we had to wait in A&E for Cécile to have a blood test, an x-ray and give a urine sample before she was installed in her room in the hematology service. The antibiotics started at 2am and as she still only had 14 000 platelets, she had another transfusion at 3am. Rather a short night, one way or another. She's not in very high spirits and is hoping she'll be allowed home on Monday if her temperature doesn't go up again.

Encore une semaine difficile. L'infirmier est passé lundi matin pour faire le bilan sanguin. Dans l'après-midi, on a reçu un coup de fil de l'hôpital nous demandant d'emmener Cécile car elle avait besoin d'une transfusion d'hémoglobine (elle était à 6g) et de plaquettes ( elle en avait 20 000 alors que le niveau habituel est entre 150 000 et 400 000). Finalement elle y a passé la nuit car il a fallu attendre jusqu'à 20h00 pour le sang et ensuite il faut compter 4 heures pour la transfusion. Elle est rentrée à la maison mardi matin, avec un peu plus d'énergie.
Elle est retournée à l'hôpital jeudi pour un bilan. Malgré la transfusion de lundi elle n'avait que 10 000 plaquettes donc elle est restée pour une nouvelle transfusion.
Elle avait l'air fatiguée vendredi mais on pensait que c'était à cause des cours et la séance avec la kiné. Mais le soir elle a pris sa température: 38,8°. Comme elle est en aplasie il fallait retourner à l'hôpital. Le Sud Francilien n'avait pas de place et du coup, il fallait aller jusqu'à l'hôpital Debré à Paris. Après un passage assez long aux urgences pour bilan sanguin, contrôle des urines et radio, Cécile a enfin pu s'installer dans une chambre au service d'hématologie. Vers 2 heures du matin les antibiotiques ont commencé et vers 3 heures elle a eu une nouvelle transfusion de plaquettes car elle n'en avait que 14 000. La nuit a été courte. Elle n'a pas trop le moral. Si la température reste stable, elle pourra peut-être sortir lundi.

Day 39/57 ... J39/57

Well we weren't really expecting to have had such a difficult week. All seemed OK last Sunday - Cécile had been well hydrated following Thursday's chemo and the weather was lovely so we took the dogs for a walk in the Forêt de Sénart. In the evening we went out for a belated birthday dinner for her brother! The doctor had given us the all clear as long as Cécile stuck to safe food. She really enjoyed her meal!

And then on Monday, back down to earth with a bump. Cécile started having headaches and nausea. We had to go to the Sud Francilien hospital for a check up and when we got home Cécile was sick. On Tuesday things got worse with a headache so bad that she couldn't sit up or stand. We had to cancel lessons and physio and Cécile stayed lying down all day. The same thing on Wednesday. She was sick in the morning and then stayed in bed all day.

On Thursday she had to go back to hospital for a check-up before her Aracytine injections which were to start in the evening (again, 4 12-hourly injections). She told the doctor about her headaches and nausea and they suspected a delayed reaction to the lumbar puncture Cécile had had the previous Thursday. On our way home Cécile was sick. She managed to sit through two classes but had to shorten the time of each class.

And then, in the evening, after dinner Cécile started being sick. From 9pm until gone midnight she was sick every 10 minutes or so. It was an awful experience for her - she was in a lot of pain and was very frightened. She was sick for the last time about half past midnight but wasn't even able to recuperate the next morning because the nurse arrived at 7h30 for her injection. Again, classes were cancelled on Friday and Cécile stayed in bed, feeling very fragile.

Saturday was steady and today, Sunday, she seems to have recovered - only very slight head pains and no nausea. Let's hope this episode won't be repeating itself. She managed to have some fun today blowing up balloons and letting her dog, Hopper, chase after them and try to catch them!

On ne s'attendait pas vraiment à une semaine aussi dure pour Cécile. Tout allait bien dimanche dernier - Cécile était bien hydratée après la chimio du jeudi 5 mars et il faisait très beau donc nous sommes allés dans la Forêt de Sénart avec les chiens. Le soir, après avoir eu l'accord du médecin et à condition de bien choisir les plats, nous sommes allés au réstaurant pour fêter l'anniversaire de Nicolas. Cécile a bien apprécié son repas.

Et puis lundi on est retombé sur terre. Cécile a commencé à avoir des maux de tête et de la nausée. Nous sommes allées au Sud Francilien pour une visite 'de routine' et de retour à la maison Cécile a vomi. Les choses se sont aggravées mardi au point où Cécile ne pouvait que rester allongée. Nous avons annulé les cours et la séance de kiné. Mercredi ce n'était guerre mieux. Cécile a vomi le matin et est restée allongée toute la journée.

Jeudi matin il fallait retourner au Sud Francilien avant de reprendre les piqûres d'Aracytine le soir (4 injections toutes les 12 heures). Cécile a parlé de ses maux de tête et de ses nausées au médecin qui pensait qu'il s'agissait des séquelles de sa ponction lombaire de la semaine précédente. En rentrant à la maison Cécile a vomi à nouveau. Elle a réussi à suivre deux cours mais en réduisant la durée de chaque cours.

Et puis le soir les choses se sont aggravées. Cécile a commencé à vomir vers 9 heures du soir et ça a duré jusqu'à minuit passé. C'était une expérience douloureuse et pénible. Les vomissements se sont calmés vers minuit trente mais elle n'a même pas pu récupérer le lendemain matin car l'infirmier est passé à 7h30 pour la piqûre. Encore une fois nous avons annulé les cours et Cécile a passé la journée au lit, dans un état très fragile.

Elle a passé une journée tranquille samedi et aujourd'hui, dimanche, elle semble avoir récupérée - seulement quelques petites douleurs à la tête et pas de nausées. Espérons que cette épisode ne se répétera pas. Elle arrive quand même à s'amuser en jouant avec le chien, Hopper, et des ballons qu'il essaie désespérément d'attraper!

Day 31 / J31 ...

Cécile was allowed out of hospital on Monday just after lunch. She was very stressed all morning waiting for the results of her blood test. Her neutrophils hadn't gone up much - she was still neutropenic (very low neutrophil count) but the number had gone from 300 to 350 so with the consent of her hematologist in Paris she was allowed to come home. The first thing she did on arriving back home was to make an 'Auntie Mary' cake (chocolate sponge cake)! That was about all she did because she was very tired - she hadn't slept too well in hospital and the emotion of wondering whether she'd be able to come home was obviously taking its toll.

She had to go back to Sud Francilien on Wednesday for a blood test to make sure the neutrophils were still going in the right direction. After a couple of hours the doctor came in with a big smile on her face and said that not only the neutrophils were going up, there were enough for the chemo to start again! The platelets were at a good level and the hemoglobin was satisfactory.

So - back in hospital on Thursday for a long day of chemo. Cécile had to have a lumbar puncture which injects three types of chemo and then the Endoxan was administered through her catheter. Because this product can cause cystitis, it was important for Cécile to be well hydrated. This was done through the catheter in hospital and then after we'd left hospital about 18h00, Cécile had to drink another 3 litres before the following lunchtime. Easier said than done. She was awake most of the night on Thursday either because she needed to wee or because I was forcing her to drink. At the same time, the Aracytine injections have started again. This is the series of 4 12-hourly injections each week for three weeks. The nurse came on Thursday evening, Friday morning and evening and the last one for this week this morning (Saturday). The injections are painful.

The list of medicine to take on Thursday evening was quite impressive: the antibiotics following her stay in hospital last week, the antiemetic to counter the day's chemo, the oral chemo which she will be taking for the next three weeks ... we have Excel printouts every month to remind Cécile which medicine she must take and when.

Friday was not a good day - hardly surprising after the previous day's chemo and a night with never more than one and a half hours between each glass of water. Classes had been cancelled as we had anticipated this state of fatigue but Cécile did manage to finish her ragdoll (see photo below)!

Just a quick explanation of the title: today, 7th March, is the 31st day of this final stage of the protocol before the two-year maintenance phase starts. It's actually more than 31 days since the stage started but if the treatment is interrupted by neutropenia or an unplanned stay in hospital, those days are not counted. Only the days of actual treatment count. This phase ends in theory on Day 57 - the 2nd April. Then, as long as Cécile is not neutropenic, the maintenance phase will start.

By the way, the ragdoll is called Bella - the protagonist of the Twilight series for the uninitiated! Cécile read the last two volumes last week in hospital!

Cécile est sortie de l'hôpital lundi dernier en début d'après-midi. Elle a stressé toute la matinée en attendant les résultats du bilan sanguin. Ses PNN n'avaient pas beaucoup augmenté - ils sont passés de 300 à 350 - mais son médecin référent à Paris à donner son accord pour qu'elle puisse sortir.

La première chose qu'elle a faite en arrivant à la maison était de faire un gâteau au chocolat! Mais son élan d'énergie s'est vite arrêté car elle était très fatiguée à cause d'un manque de sommeil et un trop-plein d'émotion.


Il fallait retourner au Sud Francilien mercredi matin pour un bilan pour vérifier le taux de PNN. En fin de matinée le médecin est arrivé avec un grand sourire: non seulement les PNN continuaient à monter, mais il y en avait assez pour redémarrer la chimio! Les plaquettes et les globules rouges étaient également à un niveau correct.

Retour donc à l'hôpital jeudi pour une longue journée de chimio. Cécile a eu une ponction lombaire dans laquelle on injecte trois types de chimio et ensuite l'Endoxan qui passait par le cathéter. Comme ce produit peut provoquer une cystite il fallait que Cécile soit très bien hydratée. Pendant la journée l'hydratation passait par le cathéter. En quittant l'hôpital à 18h00, Cécile devait boire 3 litres d'eau avant midi le lendemain. Pas si facile. Elle n'a quasiment pas dormi la nuit de jeudi à vendredi, soit parce qu'elle avait besoin de faire pipi, soit parce que je la réveillais pour la faire boire. En même temps, les injections d'Aracytine ont repris. Il s'agit d'une série de 4 piqûres à 12 heures d'intervalle chaque semaine pendant trois semaines. La dernière piqûre pour cette semaine était ce matin (samedi). Malgré les patchs anesthésiants, ce sont des piqûres qui font mal.

La liste des médicaments jeudi soir étaient longue: les antibiotiques liés à son état fiévreux, l'anti-émétique pour les chimios du jour, la chimio orale qu'elle doit prendre pendant trois semaines ... il nous faut des fichiers Excel pour rappeler à Cécile ce qu'elle doit prendre et quand.

Vendredi était un jour de repos - Cécile était très fatiguée après la chimio de la veille et une nuit quasiment sans sommeil. On avait annulé les cours prévus mais Cécile a réussi à finir sa poupée (voir photo ci-dessus).


Une petite explication du titre de cette page: aujourd'hui, le 7 mars, est le 31e jour de cette dernière phase du protocole avant que la phase d'entretien commence. En fait, plus de 31 jours se sont déroulés depuis le début de cette phase mais seulement les jours de traitement comptent. Si le traitement s'arrête pour cause d'aplasie ou hospitalisation, le compteur aussi s'arrête. Cette phase se termine normalement à J57, le 2 avril. Ensuite, à condition de ne pas être en aplasie, Cécile commencera les deux années de la phase d'entretien.

Au fait, la poupée s'appelle Bella - après la protagoniste des livres Twilight pour les non-initiés! Cécile a lu les deux derniers à l'hôpital la semaine dernière!

Back to hospital

The week started out ok - Cecile went to Sud Francilien to have her dressing changed and a blood test done. The results showed that her blood count was going down but that was to be expected after all the strong chemo Cecile has had recently. She wasn't neutropenic but not far off so we were planning to have a steady week and avoid seeing too many people.
On Tuesday she had an appointment at Debré with her hematologist. We discussed possible dates for having the central line catheter removed - probably mid-April. This will be a major milestone in the treatment as it will mark the end of the intensive treatment and the start of the two-year maintenance chemo ... and of course a return to 'normal' life: school, outings, holidays, restaurants ...
Then on Wednesday evening Cecile took her temperature and it was over 38°C. She was desperately upset as she knew this meant a return to hospital. We waited and took her temperature again but it hadn't gone down. We phoned Debré and they told us to go to Sud Francilien. Cecile was installed in her room and had had a blood test done by about 1 in the morning. The antibiotics started during the night.
Cécile's temperature didn't last too long and the doctors said that if her neutrophil count was above 300 on Saturday morning she'd be allowed home. Unfortunately this wasn't the case so she has to wait till Monday morning now when the next blood test will be taken. Let's hope her blood count will be better and she'll be allowed home for a few days' rest before the final four weeks of this part of the treatment can start.
Despite all this disappointment Cecile is still managing to keep a smile on her face!

La semaine a démarré tranquillement par une visite au Sud Francilien pour un changement de pansement et un bilan sanguin. Les résultats étaient en baisse mais on s'y attendait après la chimio intensive des dernières semaines. Cécile n'était pas encore en aplasie mais elle n'était pas loin donc on prévoyait une semaine calme sans trop de contact avec l'extérieur.
Mardi après-midi Cécile avait rendez-vous avec son médecin référent à Debré. Nous avons discuté des dates possibles pour enlever le cathéter - probablement mi-avril. Ce sera une étape importante dans son traitement qui marquera la fin du traitement intensif et le début des deux années de traitement d'entretien ... et aussi bien sûr un retour à une vie 'normale': école, sorties, vacances, restaurants ...
Et puis mercredi soir Cécile a eu de la fièvre. Elle avait plus de 38°C lors de deux prises successives et comme elle était probablement en aplasie elle savait qu'elle allait retourner à l'hôpital. Elle était très décue. Après la deuxième prise nous avons appelé l'hôpital et à 1 heure du matin Cécile était installée dans sa chambre et les infirmières ont mis les antibiotiques dans sa perfusion.
La fièvre n'a pas duré trop longtemps et les médecins ont dit que si au bilan de samedi matin le nombre de PNN était supérieur à 300 elle serait autorisée à rentrer à la maison. Malheureusement ce n'était pas le cas et elle doit maintenant attendre le bilan de lundi matin pour savoir si elle peut rentrer à la maison pour se reposer quelques jours avant de démarrer les quatre dernières semaines de cette phase. Malgré sa déception due à cette nouvelle hospitalisation, Cécile arrive à garder le sourire!

Third week of intensification 2

This week has been fairly quiet and uneventful. Monday was a long day - Cécile had three different chemos and was in hospital from 9am till 6pm. She had the usual sessions with the physio and has managed a couple of walks in the park with the dogs. As it's the school holidays we went slightly further afield for a walk taking Cécile's best friend with us. We had planned to go to the cinema again but her blood count is going down, as was to be expected, and with the flu epidemic we decided it would be more reasonable to stay at home. Cécile has been doing a lot of cooking and has started to make a rag doll - see below for her first experience on the sewing machine! Her morale is better this week although she is starting to feel more tired. For light relief we've watched a few films and this afternoon had a game of Cluedo with friends - a lot of talking and drinking of tea and not too much concentration ... needless to say Cécile won again!

La semaine a été plutôt calme. Lundi était la seule journée passée à l'hôpital mais elle était longue - Cécile devait recevoir trois chimios et elle est restée de 9 heures jusqu'à 18 heures. A part ça, il y a eu les séances avec la kiné et des balades dans le parc avec les chiens. Nous avons profité du beau temps pour aller un peu plus loin et comme c'est les vacances scolaires, la meilleure amie de Cécile a pu nous accompagner. Nous avions prévu de retourner au cinéma mais avec l'épidémie de grippe et un bilan sanguin en baisse nous avons préféré rester à la maison. Cécile a beaucoup aidé en cuisine et a commencé à coudre une poupée! La photo montre les premiers essais sur une machine à coudre! Le moral va plutôt bien même si elle commence à se sentir plus fatiguée. Pour nous distraire nous avons regardé plusieurs films et cet après-midi nous avons joué au Cluedo avec des amis - beaucoup de bavardages et tasses de thé et pas trop de concentration - inutile de dire que Cécile a encore gagné!

Second week of intensification 2

The week has been long and wearisome for Cécile. Another three long days in hospital on Monday, Wednesday and Friday. She was unusually miserable at the beginning of the week, all due to the fact that she is finding these long days in hospital more and more difficult to bear.

The corticosteroids are really taking their toll. Cécile is hungry all the time even to the point of finding it difficult to concentrate on anything else. The fact that she can't eat what she wants makes it even more difficult. Not being able to eat sugar means she can't even have any little treats in the afternoon to boost her morale. We've made more chocolate cake without sugar which works quite well and tried muffins this week too - replacing the baking powder with whipped egg-whites because of the fobidden sodium bicarbonate in the baking powder ...

The medication is also causing Cécile to have problems sleeping and she's been having some pains in her legs - not as acute as the last time back in October but enough to make her want to take some pain relief.

Apart from going to hospital, there have been the usual classes on Tuesday and Thursday and visits from the physio. On Thursday Cécile's brother took her to the book shop to choose some more books and yesterday we went to the cinema in the morning and in the afternoon some friends came round to play cards.

Cécile isn't suffering from any nausea at the moment so she has been helping out in the kitchen. We have been trying lots of new recipes - one of the days in hospital was spent combing through cookery magazines to find interesting things to eat! It has been an good experience and we now have a lot more healthy recipes in our repertoire!

This week, in theory there will only be one day in hospital. After today, Cécile has a break from chemo for nearly two weeks to give her bone marrow a rest. She will probably be very low on white blood cells in the next few days as a result of the chemo she has just been having so we need to be extra vigilant to avoid any risk of infection. There won't be any classes as the schools are on holiday for the next two weeks so Cécile can also have a break from school work.




La semaine a été longue et fatigante pour Cécile avec trois journées entières passées à l'hôpital. Elle était particulièrement malheureuse en début de semaine car elle trouve les journées à l'hôpital de plus en plus dures à supporter.

Les effets des corticoïdes se font ressentir et Cécile subit en permanence la sensation d'avoir faim au point où il lui est difficile de se concentrer sur autre chose. Ne pas pouvoir manger du sucre supprime les petits plaisirs du goûter qui aident normalement à booster le moral. Nous avons refait du gâteau au chocolat sans sucre et aussi testé des muffins cette semaine: sans sucre et aussi sans levure à cause du bicarbonate de sodium qu'elle contient! A notre grande surprise ils étaient tout à fait mangeables!
Cécile a aussi des problèmes de sommeil à cause des corticoïdes et elle a même ressenti quelques douleurs dans les jambes, pas de la même force qu'au mois d'octobre dernier mais suffisamment fortes pour qu'elle prenne des anti-douleurs.

A part l'hôpital, Cécile a eu cours comme d'habitude mardi et jeudi ainsi que des séances avec la kiné. Jeudi après-midi son frère l'a emmenée à la librairie pour acheter quelques livres et hier nous sommes allés au cinéma. Dans l'après-midi nos amis sont passés pour jouer aux cartes.

Comme elle ne souffre pas de nausées en ce moment, Cécile participe à la préparation des repas. Nous testons plein de recettes nouvelles trouvées dans les magazines de cuisine que nous avons épluchées un après-midi à l'hôpital! Nous avons ajouté des recettes saines et gouteuses à notre répertoire: à quelque chose malheur est bon!

En principe, cette semaine Cécile ne passera qu'une journée à l'hôpital. Ensuite elle n'aura plus de chimio pendant presque deux semaines afin de laisser la moelle se reposer. Elle risque de tomber bientôt en aplasie donc il va falloir que nous soyons à nouveau très vigilants pour éviter tout risque d'infection. Il n'y aura pas cours pendant les deux semaines de vacances scolaires donc Cécile pourra aussi se reposer intellectuellement.
